Completing the Evaluation of Clinical Supervision Experience and Field Placement Summary forms
To access the student's Evaluation of Clinical Supervision Experience form, click on the student's Evaluation of Clinical Supervision Experience tab and then click on the first Select button.
The form will appear and it is now ready to accept your input.

Answer all 22 questions. Choose YES or NO by clicking the appropriate button. There is also a box available for a narrative if you wish to add any additional comments.
To access the student's Field Placement Summary form, follow the same procedure, but click the Field Placement Summary tab.
Your Supervisor cannot view the student's Evaluation of Clinical Supervision Experience form.
Your Cooperating Teacher cannot view the student's Field Placement Summary form.
