Hofstra's Student Financial Aid Printable Forms page makes it more convenient for students to access forms to handle certain issues. Simply choose the form that you need, print it out, and complete it. Please follow the directions for each form as they are all different. Dependent Federal Verification Form

The Department of Education estimates they will send the FAFSA information to schools in the first half of March 2024.

Special Circumstances Assessment

Please contact the office of Student Financial Services if you have had any significant changes to your income since completing the FAFSA. Your counselor will provide a preliminary assessment and advise you of the supporting documentation required. Call 516-463-8000.

Special Circumstances may include:

  • Loss of Wages or Employment
  • Retirement
  • Loss of Disability Income
  • Loss of Benefits (taxable social security, untaxed court-ordered child support or other untaxed benefits)
  • Divorce/Separation
  • Death of a Parent/Spouse
  • Excessive Medical Expenses not covered by insurance (expenses must exceed 7.5% of your adjusted gross income as per IRS guidelines)

Aid for Part-Time Study (APTS)

To apply for Aid for Part-Time Study (APTS), complete the online TAP application (www.hesc.ny.gov ), submit your (and your parents if applicable) signed New York State Tax Returns and W2s, read and complete the APTS application, and submit it to the Student Financial Services Suite in Room 206 Memorial Hall.


GRE Fee Reduction Program

ETS provides the fee reduction program for individuals who demonstrate financial need and for national programs that work with underrepresented groups. A Fee Reduction Voucher may be used for one GRE General Test and/or one Subject Test. Voucher users pay 50% of the regular test fee. Financial Aid Frequently Asked Questions for New Undergraduate Students

This brochure will answer many questions regarding financial aid, billing, and payment and provides a month-by-month calendar of when you should do what.


Title IV Authorization for Students

Students who receive federal aid (Pell, SEOG, Stafford Loans, Perkins Loans, Grad PLUS) must complete the online form to have these funds applied to any charges other than current term tuition, fees, and room and board. For instructions on how to complete this authorization on the Portal, please visit: hofstra.edu/fundauthorization.


Title IV Authorization for Parents

Parent recipients of the PLUS Loans must complete this form to have these funds applied to any charges other than current term tuition, fees, and room and board. Complete and return to the Office of Student Financial Services in Room 206 Memorial Hall.


Winter Loan Request

To apply for a winter loan, you can complete the form or email loanoffice@hofstra.edu requesting loan funds for winter. The form can be found by clicking on Winter Loan Request above. The only loans available for the winter session are Parent PLUS for dependent undergraduate students, Grad PLUS loans for graduate/professional students, and private loans. If you are borrowing a private loan, an approved loan application must be on file before submitting the form or emailing loanoffice@hofstra.edu. If you are borrowing a Graduate or Parent PLUS loan, you must be enrolled at least half time during the winter and spring combined. Half-time enrollment is 6 credits for undergraduate students and 4.5 credits for graduate students.


Summer Loan Request

Students must complete this form if they wish to borrow federal Direct student loans for their summer 2024 registration. If you have applied for a Parent PLUS Loan or a private loan to cover your summer balance, please indicate on this form after you have registered. This form must be uploaded via the Hofstra Portal.


PLUS Loan Application

To apply for a parent PLUS Loan, please visit www.studentaid.gov. Under "Apply for Aid," click on "Apply for a Parent PLUS loan" to complete your PLUS application. You will need to log in with your FSA ID.  If you do not have one, you can create one on the login page.  For assistance, please call 1-800-433-3243.


Grad PLUS Loan Application

To apply for a Grad PLUS Loan, please visit www.studentaid.gov. Under "Apply for Aid," click on "Apply for a Grad PLUS loan" to complete your Graduate PLUS application. Note: You will need to log in with your FSA ID.  For assistance, please call 1-800-433-3243.

To request an increase in your Graduate PLUS or Parent PLUS loan, please complete and submit this form to Student Financial Services. You may submit this form on the my.hofstra.edu portal. For instructions visit hofstra.edu/fasteps.

Request for Federal Direct Unsubsidized Loan Only – FAFSA response follow-up

Your 2024-2025 Free Application for Federal Student Aid (FAFSA) indicated that your parent(s) is/are unwilling to provide their information on the FAFSA and therefore, you want to be considered for the Federal Direct Unsubsidized Loan only.
