Federal Perkins Loan Master Promissory Note
If you have been awarded a Perkins Loan, this is the Promissory Note that you must complete.
Title IV Authorization for Students
Students who receive federal aid (Pell, ACG, SMART, SEOG, Stafford Loans, Perkins Loans, Grad PLUS) must complete this form to have these funds applied to any charges other than current term tuition, fees and room and board. Complete and return to the Office of Student Financial Services in Memorial Hall Room 206.

Title IV Rescindment
Use this form (there is both a version for students and one for parents) should you wish to rescind a previous authorization you have made for the use of federal student aid. Complete and return to the Office of Student Accounts at Hofstra.

PLUS Loan Request Form
Parents should use this form to begin the application process for a PLUS (Parent Loan to Undergraduate Students) loan. Complete and return to the Office of Financial Aid.
Federal Parent Loan Data Request
FOR PARENT(S) OF A DEPENDENT UNDERGRADUATE STUDENT: Only complete and return this form if you will be a new parent borrower. This form applies only to families in which one parent borrowed a PLUS loan in a previous year and the other parent will be borrowing this year (2008-2009).
